Immunotherapy-responsive Recurrent Painful Ophthalmoplegic Neuropathy

|
Ophthalmoplegic migraine (OM) is a poorly understood neurological syndrome characterized by recurrent headaches with paresis of ocular cranial nerves. The term ¡°OM¡± was recently replaced by recurrent painful ophthalmoplegic neuropathy (RPON) due to lack of the evidence for pathophysiological link to migraine. We report a 35-year-old man who presented with three major episodes of painful ophthalmoplegia. The episodes were successfully managed by immunotherapy including corticosteroid and azathioprine. Immunotherapy is not established as a standard treatment for RPON, and only an adolescent case was reported to be treated with corticosteroid in Korea. Further studies are needed to formulate immunotherapy in RPON.
